Same-Day Restorations with CAD/CAM (CEREC)

A digital scan, computer-aided design and in-clinic milling allow inlays, onlays, crowns and implant abutments to be completed in a single visit in suitable cases.

CAD/CAM (computer-aided design and manufacturing) is a restorative method in which the tooth is scanned in 3D with an intraoral camera, and the restoration is designed on a computer and milled in the clinic. With the CEREC system, inlays, onlays, single crowns and implant abutments can often be completed in the same appointment, depending on the type of restoration and the material. As no putty impression is needed, it is also a comfortable option for patients with a strong gag reflex.

Treatment process

  1. Preparing the tooth

    Decay or an old filling, crown or bridge is removed and the tooth is prepared for the new restoration.

  2. Digital scan

    A 3D image of the tooth is captured with an intraoral camera. For small restorations this takes about 1–5 minutes.

  3. Computer design

    The restoration is designed in the software and its fit with the neighbouring teeth is adjusted on screen, in about 5–10 minutes.

  4. Milling

    The milling unit shapes the restoration from a ceramic, composite-ceramic or zirconia block. Ceramics take about 10–15 minutes; zirconia may take longer.

  5. Bonding

    The restoration is bonded directly to the tooth. Depending on polishing and finishing, this takes about 10–45 minutes.

What is CAD/CAM dentistry?

CAD/CAM stands for Computer-Aided Design / Computer-Aided Manufacturing. An intraoral camera records a 3D image of the mouth, the shape of the filling or crown is designed in the software, and the restoration is produced in the clinic from a ceramic, composite-ceramic or monolithic zirconia block.

The system has been in use since 1985 and took its present form through research at the University of Zurich. In its early years the equipment was bulky and very costly, which limited its use; technological progress has since made it far more practical for everyday clinical work.

Why a digital impression is more comfortable

No putty impression material is placed in the mouth. Instead, a small camera is moved over the teeth and the 3D data are transferred to the computer, which is particularly helpful for patients with a strong gag reflex.

  • Errors caused by an impression stretching as it is removed, or by air bubbles or saliva trapped inside it, are avoided.
  • Transfer errors that can occur when an impression is sent to a laboratory and modelled are reduced.
  • Because the restoration is made quickly, temporary fillings or crowns are usually unnecessary, so there is no risk of a temporary coming loose or breaking.
  • When treatment is completed in one visit, no further anaesthesia is needed for follow-up appointments; in many cases the work is finished before the anaesthetic wears off.

Restorations that can be made with CAD/CAM

Inlays and onlays (ceramic fillings)

Large cavities involving two or three surfaces of a tooth are difficult to restore with conventional fillings; if the matrix band placed around the tooth does not fit well, food can become trapped between teeth or the gum may become swollen. In teeth with little healthy structure left, CAD/CAM inlays and onlays can, in suitable cases, avoid the need for a crown or even an extraction.

Crowns

A single crown can be placed in approximately 30–60 minutes.

Bridges and veneers

Bridges and porcelain veneers can also be scanned and designed digitally. More detail is given in the prosthodontics section.

Implant abutments

In aesthetically demanding areas, the connecting part that fits into the implant, known as the abutment, can be made to the length and width that suit the patient's mouth. Avoiding repeated abutment try-ins helps protect the surrounding gum and bone.

Materials

CAD/CAM restorations are made from metal-free, tooth-coloured materials such as Empress, e.max and monolithic zirconia. These materials are biocompatible and are also suitable for patients with metal allergies.

  • Empress and e.max contain neither metal nor zirconia, so they transmit light more like natural teeth and offer a highly natural appearance.
  • Zirconia is very strong and straightforward to use, although it is less translucent than glass ceramics.
  • Being metal-free, the restorations allow the tooth underneath to be assessed more easily on future X-rays.

Where a metal-framework porcelain bridge is preferred for a very long span, the impression can still be taken digitally. The precision of digital scanning and laser-sintered metal frameworks considerably shortens the try-in stages.

How long does it take compared with conventional methods?

Porcelain fillings, inlays and onlays, single crowns, veneers, zirconia work and bridges usually take 3–7 days with a conventional laboratory process. With CAD/CAM, the same work can be completed on the same day, sometimes within 1–2 hours, or on the following day, depending on the type of restoration and the material.

Frequently asked questions

Can a CAD/CAM restoration really be finished in one visit?

In suitable cases, restorations such as single crowns, inlays and onlays can be completed in the same appointment; a single crown takes about 30–60 minutes. Zirconia and multi-unit work can take longer and may be finished the next day. The exact timing is confirmed after your examination.

I have a strong gag reflex. Will the impression be a problem?

No putty material is placed in the mouth with CAD/CAM; the teeth are scanned with a small intraoral camera. Most patients with a gag reflex find this considerably more comfortable.

Which materials are used?

Glass ceramics such as Empress and e.max, and monolithic zirconia. All are tooth-coloured and metal-free. The choice depends on the position of the tooth, aesthetic expectations and the biting forces involved.

When is an inlay or onlay recommended?

An inlay or onlay may be recommended when a large part of the tooth has been lost to decay or an old filling, but not so much that a crown is needed. It allows more of the healthy tooth structure to be preserved.

I am allergic to metal. Is CAD/CAM suitable for me?

The ceramic and zirconia materials used are metal-free and biocompatible, so they are generally suitable for patients with metal allergies. Please mention any allergy during your consultation.

I need several treatments, but my time is limited. What can we do?

This is a very common situation. With an accurate diagnosis and careful planning, and provided appointments go ahead as scheduled, treatment time can be shortened considerably. In suitable cases, completing root canal treatment in a single day, making crowns and ceramic inlays in the same session with CAD/CAM, and placing implants with X-Guide all help. While one side of the jaw is anaesthetised, treatments in that area such as fillings, deep cleaning, root canal treatment, implants and extractions can also be done in the same session.

Food keeps getting stuck between my teeth even after a filling. Is there a solution?

If the cause is bone loss, the problem can only be managed with floss or interdental brushes. If the cause is a very large filling that doesn't make good contact with the neighbouring tooth, a ceramic inlay or onlay made with CAD/CAM may be a solution. Which applies to you is determined by an examination and X-rays.
